How does a Malcolm Baldrige Quality Award-winning healthcare system adapt to the changes of healthcare reform? At Mosaic Life Care (formerly Heartland Health), the answer was to go all-in on population health by adopting a “patient-centered optimal health model.” The transformation began with a brutally honest self-evaluation that revealed the scope of the effort required to achieve the goals of “improving the patient experience, improving the health of the population, and reducing the per capita cost of care.” In this program, Mosaic Life Care leaders discuss how they are enabling the clinical transformation by building on core strengths, employing a Shared Savings ACO, and successfully expanding their vision to fully embrace a more expansive version of care.
About our host:
Mosaic Life Care is an integrated health delivery system, which includes a medical center, clinics, Heartland Foundation and Community Health Improvement Solutions. Founded in St. Joseph in 1984, the organization seeks to improve the health of area individuals and communities and to provide the right care, at the right time, in the right place, at the right cost with outcomes second to none. Commitment to this mission has enabled Mosaic Life Care to become the leader in health care in a 21-county area of northwest Missouri, northeast Kansas and southeast Nebraska. Providing exceptional care and service in an honest, respectful, trustworthy and caring manner is simply the way Mosaic Life Care operates.
